Obstructive sleep apnoea was long treated as a plumbing problem — a blocked pipe to be propped open. This review reflects a more sophisticated modern understanding: that OSA is multi-factorial, driven by a combination of how easily the airway collapses, how well the throat muscles respond, how readily a person wakes, and the stability of their breathing control (the "loop gain").
